Indications: sterk vermoeden op- of bewezen cefalosporine-en meropenem-allergie of een eerdere ernstige 'delayed type' allergie op een ß-lactam antibioticum

Vancomycine dosering en TDM in overleg met ziekenhuisapotheker

General comments

Behandelduur S. aureus bacteriemie altijd in overleg met lokaal A-team, dan wel internist-infectioloog en/of arts-microbioloog.

In geval van een S. aureus bacteriemie altijd starten met 12g/24 uur. Zodra na een formele work-up, in samenspraak met het A-team, er sprake blijkt van een ongecompliceerde S. aureus bacteriemie kan worden gekozen de dosering flucloxacilline te verlagen naar 6g continu per 24 uur.
