clindamycine po 600mg 3dd

General comments

Totale therapieduur ten minste 6 weken. Switch naar oraal is afhankelijk van kweekresultaat, klinische respons en aan- of afwezigheid van bacteriëmie.

Zonder bacteriëmie:

  • Ten minste 1 week intraveneuze behandeling.
  • Bij gebleken gevoeligheid zodra goede klinische respons switch naar orale therapie met clindamycine 600 mg 3dd oraal. Bij clindamycineresistentie zijn cotrimoxazol 960 mg 2dd oraal of flucloxacilline 1000 mg 4dd oraal een alternatief.

Met bacteriëmie: zie sepsis o.b.v. S. aureus (sepsis - S. aureus en stafylokokken | CZEMMC (antibiotica.app))

  • Tenminste twee weken intraveneus behandelen in hogere dosering en afhankelijk van andere strooihaarden langer (bijvoorbeeld endocarditis), altijd in samenspraak met lokaal A-team, internist-infectioloog en/of arts-microbioloog.
