General comments

Deze adviezen zijn bedoeld voor patiënten met neutropenie als gevolg van chemotherapie (niet bij neutropene patiënten met koorts door ziekte (bijv. MDS of aplastische anemie, CRS door CAR-T) of niet-chemotherapeutische medicatie (bijv. venetoclax, hypomethylerende middelen)).

Definities:

  • High risk neutropenie: absolute neutrofielen <0.5x109/L (of verwacht <0.5x109/L binnen 48 uur) voor >7 dagen;
  • Standaard risk neutropenie: absolute neutrofielen <0.5x109/L (of verwacht <0.5x109/L binnen 48 uur) voor ≤7 dagen. Bij standaard-risico neutropenie hangt het advies omtrent antibiotica af van de klinische burden en ernst van ziekte en daarmee afhankelijk van de Multinational Association for Supportive Care in Cancer (MASCC) score of een vergelijkbaar instrument. MASCC score berekenen: MASCC Risk Index for Febrile Neutropenia - MDCalc.
    • Hoge MASCC score (= laag risico op ernstige complicaties, ≥21);
    • Lage MASCC score (= hoog risico op ernstige complicaties).

Additionele overwegingen:

  • Empirische behandeling van gist infecties moeten beperkt worden tot een setting met een hoge lokale incidentie van invasieve gistinfecties bij patiënten met een hoge burden van ziekte (m.a.w. IC opname, enterocolitis), in combinatie met persisterende fungal spp in SDD kweken EN/OF indien patiënten geen antifungale profylaxe hebben gekregen;
  • Bij een centrale lijn in situ (CVC) kan behandeling van coagulase negatieve stafylokokken en Enterokokken worden overwogen bij neutropene patienten op de IC EN/OF bij positieve bloedkweken. Bij hemodynamisch instabiele patiënten die opgenomen worden op de IC kan overwogen worden vancomycine te starten bij patiënten met een CVC voorafgaand aan het ontwikkelen van koorts;
  • Neutropene enterocolitis moeilijk te onderscheiden van C. difficile wat derhalve ook altijd moet worden onderzocht.
